New patients

Please arrive at the office 5-10 minutes before your appointed time.  Bring current as well as older eyeglasses and prescriptions.  Contact Lens wearers please wear your contact lenses in (at least 2 hours, please) and bring all available info: current contact prescriptions (brand, base curve, powers for both eyes), boxes/packaging, and notes from previous eye doctor.

Returning patients

Please arrive at the office 5 minutes before your appointed time.  Bring your eyeglasses.  Contact Lens wearers: please wear your contact lenses in (at least 2 hours, please.)

Pre-Laser Surgery  Appointment patients

Please arrive at the office 5 minutes before your appointed time.  Bring your eyeglasses.  NO soft contact lens wear for the 7 DAYS leading up to the appointment.  Rigid contacts - Dr. Folkers will advise.  No eye make-up that day, please.  Your eyes will continue to be dilated hours after the appointment, this may make work and driving more difficult - please plan your schedule accordingly.

Cancellation Policy

Your time is important.  We want you to understand that time at our office is valuable as well.  Appointments not cancelled/rescheduled 24 hours or more before the appointed time will be charged a fee of $120.

(for appointments the day after a holiday: cancellation/reschedule before close of business the day BEFORE the last business day before the holiday)
